OSAS’’lı hastaların hastalık şiddeti ve tedavide ihtiyaç duydukları basınçlı hava desteği bir çok faktör tarafından etkilenmektedir. Obezite, alkol kullanımı, gündüz aktiviteleri, ek hastalıklar, nazal konjesyon, yatış pozisyonu, uykunun evreleri, gibi faktörlere göre bu ihtiyaç azalıp artmaktadır.
Non – REM evresinde, vücudun üst kısmının yüksek olduğu durumda, lateral pozisyonda ihtiyaç duyulan CPAP basıncı % 50 oranında azalır. Alkol alındığında ise CPAP basıncı yetersiz kalabilmektedir. Yani optimal CPAP basıncı geceden geceye hatta bir gece boyunca saatten saate değişkenlik gösterebilmektedir. Bu nedenle; ototitrasyonlu CPAP’lar, evde uzun süreli CPAP tedavisi için de önerilebilmektedir.
APAP’ta sabit bir basınç yoktur. Basınç zaman zaman apne oluşumunu engellemek için yükselir. Gece boyunca basınç değişir. Bu aletle gerekmediği sürece düşük basınç uygulandığı için yüksek basıncı tolere edemeyenlerde kompliansın arttığı gösterilmiştir. Olumlu etkilerinin tersine bazı otörler aletin, apne geliştikten sonra artan rezistansı algılayarak basıncı yükselttiğini dolayısıyla yeterince etkin olmadığını savunmaktadırlar. Sabit basınçlı CPAP’a göre yeterli avantajı gösterilememiştir. Uzun süreli kullanımda sonuçları bilinmemektedir. Pahalı olması da dezavantajdır.
